Should I start over?

I took a long break from this game. I stopped playing just before episode 4 came out. Should I just play episode 4 or start from the beginning? Have they added anything new to episodes 1-3 in the last 2 years?

You can certainly start over for a refresher on the mechanics, shake off the cobwebs, that sort of thing. Maybe also to experience the story again, although it gives you a brief recap at the start. But there's no inherent disadvantage to starting right at episode 4. Will's inventory doesn't carry over, and it doesn't appear that your decisions matter from one episode to another either.

Without spoiling the episode itself, I will say this: Episode 4 is a test on just about everything you have learned up to this point, plus a few new curveballs.

Episode 4 is by far the hardest episode in Wintermute. So a restart to get back into the swing of it is not a bad idea.

But it's up to you really.

It's really your preference.  The game saves, so it's not like you will experience perma-death.

No, nothing had been added to Eps 1-3 in the last two years I'm pretty sure.
