Punishing colision with objects?

I don`t know...this is more a kind of suggestion,than wish.Even more question,than suggestion.

I just wonder,if there would be possible to add mechanics in game,which will make our character gets hurt after colision with objects.Like when he hits tree whille running or run into a rock,not abble to stop his running in time.Or indoors if he run downstairs,crash into walls or furniture.

I know there is already punishment as sprained ankle,but I wonder,if there it could be other sort of injuries and how hard is to implement that mechanics into existing ones.And,main thing: if adding those would be in game concept at all.
