Wear and Tear

It's accelerated for game purposes [likewise with food decay and other supplies]... Otherwise if items were to last as long as IRL then you would only need to find one item, and there wouldn't really be any reason to explore to scavenge for supplies, or spend the time crafting.

They could achieve the same effect, but with more realism if the degradation was explained some. For example, every so often walking (particularly in wooded areas) the character could say "damn, caught a branch." Degradation due to wolf attack is already one way they do it well. Perhaps in blizzard your scarf might just blow away.

Instead of a general slow decline, make it more like getting sprained ankles. Walk around through tough bush country and in bad weather, and the wear-and-tear events pile up faster.
