Be able to optionally choose any of the Cheat Death afflictions and penalties at the start of a game in any difficulty including Misery. I know this can essentially be done now if you die and cheat death right away in any difficulty except Misery. But I suspect there are some players out there that would welcome an even more punishing experience with a choice of optional additional afflictions and penalties right off the bat in Misery. It would also streamline the process for the lower difficulties to optionally just pick them right out of the gate. This is just a suggestion, and I know the game is fine without this. It was just a thought that crossed my mind when I viewed a YouTube video of someone planning to use all three Cheat Death afflictions stacked to start out a new game.
