Side quests not triggering

Episode 2 - the Lake Gunshots side quest didn't trigger at all this time.  Do the quests always trigger (assuming you do the same things)? Or are they a sometimes yes, sometimes no thing?  Also, the phone didn't ring this time.  So that conversation bit didn't trigger.  Basically - is this a bug or a feature?

And now both have shown up.  Weirdness.

I'd have to replay this, but I also seem to recall never being able to finish the Gunshot questline (seemed to always point me to the body on the lake).

once you find the body you should be able to follow the blood splotches and talk to a door, well technically someone on the other side of the door.
