How the hell do you get into the custom difficulty menu?  Durned if I can find it, or do I have to start a new survivor?

The Custom settings are decided when you start a new Survival Mode game - you'll see the option after you hit "New" in the menu. From there you can adjust what you'd like, and then start the game. You can't adjust the settings again once you've started. Hope that helps!

I sort of suspected that, but wasn't sure.
